How is FPIES typically diagnosed?

 

FPIES can be hard to diagnose. Blood allergy tests can help reveal IgE-mediated food allergies, but FPIES is a non-IgE-mediated disorder. So far, there is no valid laboratory or skin test for FPIES.

Furthermore, FPIES reactions are often mistaken for something else, like a stomach bug. The lag time between feeding and the onset of symptoms can cloud connection between cause and effect. Often, FPIES is only suspected after multiple allergic reactions.

With no lab test to identify FPIES, it must be diagnosed the classic way: with an interview and physical exam by a doctor. An allergist or pediatric gastroenterologist will take a detailed history of the symptoms, including foods eaten and a timeline of reactions.

The onset of acute symptoms after a history of chronic symptoms confirms the diagnosis of chronic FPIES — which is often only diagnosed looking back or after ruling out more common digestive problems.

Once FPIES is suspected, an oral food challenge may be performed under medical supervision to confirm the diagnosis.

In an oral food challenge, the healthcare team gives a patient the suspected food, waits and watches carefully. If the symptoms come back, the test is positive. For a food challenges to work, the trigger foods need to be removed from the diet first and all symptoms resolve before reintroduction.
